Patient Support

Cognitive changes can be a difficult diagnosis for both the patient and their family members. At the Memory Care Center, we advocate for our patients’ best interest and always include care partners as part of the treatment team.

A cognitive diagnosis is not always definitive, and it may take several months to get to the root cause of cognitive changes.The experts at the Memory Care Center will do everything to help guide you and your care partner through this process and provide as much emotional support as possible.


Memory Care Support Includes:

Counseling: Our team at the Memory Care Center includes medical professionals from geriatric psychiatry, clinical social work and neuropsychology. These behavioral health specialists offer education and support to patients and their care partners.

Classes and Support Groups: There are numerous free, virtual webinars offered by Hartford HealthCare that address neurodegenerative conditions such as dementia.
