As you get ready for your first visit with one of our Ayer Neuroscience Institute headache experts, we know you probably have questions.

Our goal is to ease your mind by keeping the process easy, answering any questions you may have, and offering you the many support services we have in place.


Who We See

We can only see patients age 18 and older with a referral. Please talk to your primary care provider for a referral if you experience daily, routine, periodic, or occasional headaches.

We currently do not see the following patients:

  • We do not accept self-referrals - a referral from a primary care physician is required.
  • Patients who are under the age of 18 and/or in high school.
  • Patients who have an active workers' compensation claim related to headaches or migraines.
  • Patients who experience headaches or migraines due to a motor vehicle accident.

Scheduling an Appointment

Here are four simple things we ask from any new patient who schedules an appointment with us. Once we receive this information, our new patient coordinator will contact you to schedule an appointment.

Before we can schedule an appointment, we ask new patients to fill out our online headache questionnaire. Our headache specialists use the information from this tool to help design a personalized care plan for you, ensuring you receive the best possible consultation with us.

Fill out the new patient forms.

We will ask you to review and sign this patient treatment agreement form when you arrive. This will help you understand your important role in your care and help us preserve the provider-patient relationship.

Send your medical records.

We need your medical records, progress notes, recent labs, and recent imaging at least three days before your new patient visit so the specialist can review them and help create your treatment plan. Your referring provider can send them with the referral, too.

Use the headache diary.

If possible, start using this headache diary before your first visit. Many patients find that keeping track of their headaches and day-to-day information in this diary helps find an effective treatment. You should also bring your diary to each appointment.

Your First Appointment

Please arrive prior to or at your scheduled visit time to complete additional important paperwork. If you must cancel or reschedule, we ask that you call at least 24 hours in advance. Please understand that if you do not call to let us know you will not be coming for your first appointment, we will not be able to reschedule you. Please bring your filled-in headache diary to the appointment.

Cancellation Policy:

Your care is important to us and we want to ensure that our headache specialists are available to help you. For that reason, we want to let you of our cancellation policy to avoid any disruption in your care. To provide the best care and continue treatment at the Headache Center:

  • All patients should be seen at least once a year.
  • Established patients who do not show for an appointment or call to cancel will be discharged from the practice after two missed appointments in a calendar year. Canceling or rescheduling must be done at least 24 in advance of the appointment by calling or through MyChart message.
  • New patients who do not show for an appointment or call to cancel will not be rescheduled.
