The structure of the Ayer Neuroscience Institute team fosters a collaborative environment and the providers at the Epilepsy Center appreciate being able to work with other experts to create a treatment plan that benefits the patient.

This includes collaborating closely with referring providers on the shared goal of improving the patient’s health and sense of independence.

Referrals

To be seen at the Ayer Epilepsy Center, your neurologist or primary care provider must send a referral to our clinic in one of the following ways:

  • If you work within Hartford HealthCare, use the Ambulatory Referral to Neurology-Epilepsy
  • From outside Hartford HealthCare, fax a referral to 860.545.5003

First Seizure Clinic Referrals

Hartford HealthCare providers:

  • Search in Epic for Ambulatory Referral to Neurology - Epilepsy Center
  • Select the “First Seizure” button

External Providers:

  • Fax referral to 860.545.5003
  • Specify “First Seizure Clinic”
  • Include patient insurance information
  • Include recent clinic note referencing the seizure event: Call 860.972.3621
Epilepsy Fellowship

The University of Connecticut/Hartford HealthCare Epilepsy Fellowship is an ACGME accredited one-year program, which is primarily based at Hartford Hospital, a NAEC designated level IV epilepsy center.
